Relief of chronic epiphora by transantral vidian neurectomy.

چکیده

One of the constant side-effects of a successful transantral vidian neurectomy, well-known among otolaryngologists, is the interruption of the secreto-motor nerve supply to the lacrimal gland. Since this secreto-motor nerve supply is concentrated in the nerve of the pterygoid canal (vidian nerve) this is only to be expected. This depression of lacrimation appears to be permanent, yet in some 300 recorded operations done over the last 7 years (Golding-Wood, I962; Hiranandani, I966; Gregson, I969) there has been no evidence of a subsequent keratoconjunctivitis sicca. It appears that the goblet cells of the conjunctiva take over and provide the eye with sufficient lubrication (Golding-Wood, I964). The occurrence of a severe case of unilateral epiphora after a major injury to the eye and lacrimal apparatus seemed to be a good opportunity to investigate the value of this side-action of vidian neurectomy, for, from the rhinological viewpoint, the patient had nothing to lose, and from the ophthalmological viewpoint there might be much to gain.

Endoscopic Vidian Neurectomy: The Anatomy Consideration and Preoperative Images Analysis

Although surgery is not a primary treatment for manaing rhinitis, it can have some adjuvant benefit in patients with significant nasal congestion or poor pharmacologic response. It is usually performed by surgical approaches to the inferior turbinate, and the more aggressive procedures such as the vidian neurectomy have not been widely applied. Traditional approaches to the vidian canal necessi...

Long-Term Effectiveness and Safety of Endoscopic Vidian Neurectomy for the Treatment of Intractable Rhinitis

OBJECTIVES Vidian neurectomy could be considered the treatment of choice for intractable rhinitis, because it is the only method that can permanently block the pathophysiological mechanism of rhinitis. The goal of this study was to evaluate the effect of vidian neurectomy on nasal symptoms and tear production, and to assess for possible complications. METHODS Six patients with intractable rhi...

Endoscopic vidian neurectomy assisted by power instrumentation and coblation.

Vidian neurectomy has been used to manage intractable vasomotor rhinitis for decades. After the introduction of endoscopic sinus surgery in the 1980s, transnasal endoscopic vidian neurectomy (EVN) was subsequently reported. The most common problem in performing EVN was excessive bleeding from the pterygopalatine fossa. The complexity and vascularity of the pterygopalatine fossa can cause bloody...

The anatomic analysis of the vidian canal and the surrounding structures concerning vidian neurectomy using computed tomography scans.

INTRODUCTION The type of endoscopic approach chosen for vidian neurectomy can be specified by evaluating the vidian canal and the surrounding sphenoid sinus structures. OBJECTIVE The variations and morphometry of the vidian canal were investigated, focusing on the functional correlations between them which are crucial anatomical landmarks for preoperative planning. METHODS This study was pe...
